Care Management
Care Management is a team-based approach to patient care that focuses on the clinical and social needs of the patient. It encompasses care coordination, self-management support and outreach tools. As an NCQA accredited Patient-Centered Medical Home, BCH strives to take care of the whole patient, to help them achieve the health goals they have set for themselves.
Our Care Management staff is instrumental in achieving this goal by providing expertise in health education, access to community resources and assistance with coordination of care strategies. This is a free service available to all BCH patients and their support systems.

- Assistance with managing chronic diseases i.e. Diabetes + Hypertension

Women, Infants and Children Program (WIC)
The Maryland WIC Program is a nutrition program for woman, infants and children that gives extra food, tips on healthy eating, breastfeeding help and referrals for other services. From November 26, 2024, to February 25, 2025, a representative will be onsite in our Lothian office, one Tuesday a month, from 9:00 am – 3:00 pm. Confirmed dates are listed below.

Birth to Five Referral Program (B-5)
Bay Community Health has partnered with the Anne Arundel County Public Schools Birth to 5 Program. This program provides free diagnostic and intervention services for residents of Anne Arundel County. These services are provided to all children who are demonstrating qualifying delays or disabilities for educational services. Parents and guardians can complete a referral for an evaluation to determine if their child is eligible for free early intervention services. Anne Arundel County Mobile Food Pantry
The Anne Arundel County Mobile Food Pantry operates at various locations throughout the county, giving access to free food options to those in need. For the months of November and December, Bay Community Health is partnering with Shore United Bank (Lothian) as the site for the mobile food pantry. All are welcome and Identification is not required.

Though we participate with a variety of insurance carriers, we understand circumstances may vary from person to person. We are here to help. In the event you are not insured or having difficulties paying your bill, alert a Patient Service Representative and they will direct you to a member of our Care Management or Accounting staff for additional assistance. Our dedicated staff may be able to assist you with applying for insurance, our sliding fee discount program or arranging a payment plan.

Youth Mental Health First Aid
Youth Mental Health First Aid is designed to teach parents, family members, caregivers, teachers, school staff, peers, neighbors, health and human services workers, and other caring citizens how to help an adolescent (age 12-18) who is experiencing a mental health or addictions challenge or is in crisis. Youth Mental Health First Aid is primarily designed for adults who regularly interact with young people. The course introduces common mental health challenges for youth, reviews typical adolescent development, and teaches a 5-step action plan for how to help young people in both crisis and non-crisis situations. Mental Health First Aid
Mental Health First Aid for Adults teaches people how to recognize signs of mental health or substance use challenges in adults ages 18 and older, how to offer and provide initial help, and how to guide a person toward appropriate care if necessary. Smoking Cessation

Wellness Wednesday
Bay Community Health's weekly virtual program is catered to parents and caregivers of children. Dr. Court is a subject matter expert who focuses her discourse on youth and their families. Topics will vary, but some of her recent themes have covered anxiety + depression, student-athletes, communication skills, and parenting style(s).


Free Backpack Program
Our Care Management team offers a free backpack, filled with school supplies to all school-age children who complete an annual wellness visit at Bay Community Health. This is a free service available to all eligible patients, regardless of their insurance carrier.
School sports are also back in session, many of which require participants to have a recent sports physical by a licensed provider. Sports physicals differ from annual wellness visits and are often a separate appointment. Visit your local public school district’s website to access the school health, sports physical forms and more for your child’s appointment.

Newborn Welcome Gift Program
Bay Community Health sees patients of all ages. That includes newborns. When newborn babies come in to establish care, they are welcomed with a gift from our health center. Generally, the gifts consist of thermometer, burp cloth, bib, toy and a blanket. Samples of other baby products may also be provided based on availability.

Seasonal Flu Vaccine Clinics
In addition, our staff runs a variety of health education classes and participates in health fairs and wellness events in the communities we serve. Bay Community Health holds a Free Flu Clinic for the community at large while supplies last.

Free Naloxone Training and Training Kits
Bay Community Health (BCH) is an Opioid Response Program authorized by The Maryland Department of Health, Center for Harm Reduction Services to conduct Naloxone trainings and dispense Naloxone kits.
As part of our ongoing efforts to promote health and safety within our community, we are reaching out to the {ENTER AGENCY NAME} to offer Naloxone training and/or kits to your agency.
This training aims to equip community members with the knowledge and skills to administer Naloxone, a life-saving medication that can reverse opioid overdoses.
Opioid overdoses continue to be a significant public health concern, and having Naloxone readily available can make a crucial difference in saving lives. By attending our training, participants will learn how to recognize the signs of an opioid overdose and how to properly administer Naloxone.
These trainings and kits are available free of charge, and are open to all community members, regardless of prior medical experience. Whether a concerned citizen, a community leader, or someone personally affected by opioid misuse, your participation can make a meaningful impact.
