Financial Assistance
Though we participate with a variety of insurance carriers, we understand circumstances may vary from person to person. We are here to help.

In the event you are not insured or having difficulties paying your bill, alert a Patient Service Representative and they will direct you to a member of our Care Management or Accounting staff for additional assistance. Our dedicated staff may be able to assist you with applying for insurance, our sliding fee discount program or arranging a payment plan.

Insurance Eligibility
Bay Community Health has 3 Certified Application Counselors onsite to assist with enrolling in a health insurance through the Marketplace, Medicaid/Medicaid, or other programs. Our Counselors will work with you one-on-one to assess your federal, state and local benefits. You do not have to be a patient to access this service. Appointments are preferred – call our West River office to schedule an appointment or speak with an Application Counselor.
Payment Plans Available
Bay Community Health prides itself with providing access to quality medical and behavioral health care regardless of our patient’s financial circumstances. If you are not eligible for insurance or having difficulties paying your bill, we offer payment plans specific to each patient’s needs. Sliding Fee Program
As a Community Health Center, we offer a Sliding Fee Program based on household income and family size, which reduces the amount you pay for health care services. If you qualify, you may pay a reduced cost for most services. You may be eligible for this program even if you have insurance. Applications are available at the reception desk, at each location. All information on the application is kept confidential. We have designated staff available to help you with completing these applications.
Federal Poverty Guideline will be used for the Sliding Fee Program. If your income falls within the guidelines, we encourage you to apply.
Proof of income is required to process your application. A copy of your most recent Federal Income Tax Return is strongly encouraged as proof of income. However, the documents listed below are also acceptable:
W-2 forms
Income tax returns
Social Security notice
Current pay stubs
Unemployment award notice
Letter from employer
