In the event you are not insured or having difficulties paying your bill, alert a Patient Service Representative and they will direct you to a member of our Care Management or Accounting staff for additional assistance. Bay Community Health has Certified Application Counselors onsite to assist with enrolling in a health insurance through the Marketplace, Medicaid, or other programs. Our Counselors will work with you one-on-one to assess your federal, state and local benefits. You do not have to be a patient to access this service. Appointments are preferred – call our office to schedule an appointment or speak with an Application Counselor.

Federal Poverty Guideline will be used for the Sliding Fee Program. If your income falls within the guidelines, we encourage you to apply.

As a Community Health Center, we offer a Sliding Fee Program based on household income and family size, which reduces the amount you pay for health care services. All information on the application is kept confidential. We have designated staff available to help you with completing these applications.

Proof of income is required to process your application. A copy of your most recent Federal Income Tax Return is strongly encouraged as proof of income. However, the documents listed below are also acceptable:

  • W-2 forms
  • Income tax returns
  • Social Security notice
  • Current pay stubs
  • Unemployment award notice
  • Letter from employer
