Accepted Insurances
Bay Community Health participates with a variety of insurance plans. To confirm we have the most current information, we ask for your insurance card and a photo ID at each visit.

In the event you are not insured or having difficulties paying your bill, we can help! Please alert a Patient Service Representative and they will direct you to a member of our Care Management or Accounting staff for additional assistance. Our dedicated staff may be able to assist you with applying for insurance, our sliding fee discount program or arranging a payment plan
